The incision used in open rhinoplasty is an extension of the closed rhinoplasty incision. Most of the incision is inside the rim of the nostril where it can't be seen. A small section of the incision is placed in the columella (the fleshy portion of skin between the nostrils - this is called the columellar incision). The columellar incision is strategically designed in a z shape so that it is almost imperceptible when healed.
