Hyperpigmentation is characterized by the darkening of an area of skin caused by the overproduction of melanin – a pigment produced by cells called melanocytes.

INTRODUCTION

Let’s face it: the freckles that looked so cute sprinkled across your nose in your teens and 20s may, by your mid-to-late-30s, have lost most of their appeal. That's because, over time, cumulative sun damage can cause freckles, brown spots and other marks to increase and darken, leaving skin looking blotchy and aged. It’s due to a process called hyperpigmentation – harmless in most cases but, cosmetically speaking, less than desirable. Luckily, there are many options for preventing and treating brown spots, age spots, liver spots and other marks safely and successfully. How Hyperpigmentation, Sun Spots and Age Spots Develop

Some people may possess a high concentration of melanocytes due to hormonal changes or a pre-existing medical condition, a far more common cause of hyperpigmentation is sun exposure, which causes hyperactivity in these cells.

What should I expect with a Forever Young BBL treatment?

Forever Young BBL treatments are relatively quick, with minimal pain or discomfort and with little to no downtime. Our BBL Hero upgrade allows us to apply the device to large areas of the body, allowing you to be in and out of this treatment during a lunch break.

Many patients are comfortable carrying on with their daily routine after their BBL sessions, but keep in mind that you’ll be more sensitive to sunlight for several weeks, so ensure you are always wearing full-spectrum sunscreen and avoid direct sunlight as much as possible.

You can expect a slight amount of redness in the treated area, which will only last a short time—sometimes only a matter of hours. What should I expect with a MOXI laser treatment?

Immediately after the treatment, it will feel and look as if you have a mild sunburn feeling for approximately one hour.

The next day you will experience some mild pink flushed skin, which is easily covered with mineral makeup. Your skin will feel hot, but shouldn’t be uncomfortable or painful.

From day 3 onwards, your skin will transition to a very dry texture for approximately 5 days (other people generally won’t even notice it). By the end of this one-week period, your skin will feel smoother, and you will notice a healthy glow.

While you’ll definitely see improvements after your first treatment, we often recommend 3-6 sessions, spaced every 4 weeks for the best results. We offer packages of x3 MOXIs. Your treatment provider will be able to suggest the best course of action with MOXI, and will suggest how many treatments are ideal to achieve your results.

How is MOXI laser performed?

When you arrive for your MOXI laser session, a topical anesthetic will be applied to the entire treatment area to numb the skin. Once the treatment begins, the MOXI laser handpiece is rolled over your skin, sending its non-ablative wavelengths to the bottom layers of the skin and tissue to stimulate collagen from within.

How does MOXI laser work?

MOXI uses a non-ablative laser, meaning that it doesn’t resurface the skin like other lasers (HALO, ProFractional, TRL). This is what makes MOXI the perfect year-round treatment for most skin types. MOXI works by heating up the tissue underneath the dermis to stimulate collagen without touching the surface.

Who is a candidate for MOXI laser resurfacing?

While MOXI is absolutely custom-built to treat most skin types, at all times of the year, there are some groups of patients who may find MOXI the ideal treatment:

What should I expect with a Contour TRL treatment?

Our skin specialists have performed thousands of laser treatments and are experts in helping you prepare and recover from your procedure. You will be provided with detailed instructions prior to treatment, such as what to avoid and how to prepare your skin.

The treatment itself takes roughly 30-45 minutes and you will be asked to avoid certain stay out of the sun post-treatment. After the treatment, you will be provided with comprehensive recovery instructions, with details on how to keep your skin clean and protected as you recover. It is also recommended to return to the clinic at certain intervals to receive a red light therapy treatment, which will help to enhance and speed up the healing process.

The amount of downtime that you experience will depend on how deep your treatment is and what areas are treated, which will be discussed with you during your consultation. Immediately after the treatment, your skin will be red, feel sunburned, and potentially experience some pinpoint bleeding. We do recommend taking a few days off work, and to avoid exercise and exposure to the sun. Most patients having deep skin resurfacing will take 5 to 7 days off work and limit social activities for 1 to 2 weeks.

How is Contour TRL performed?

While this is an incredibly powerful laser treatment, it is also regarded as the safest and most effective laser for gently and precisely removing the outer layers of the skin. The result is a reduction or even elimination of fine lines and wrinkles, and softer, smoother, younger-looking skin.

A numbing topical ointment will be applied to your skin prior to treatment, as well as other methods of pain management will be discussed prior to your appointment. Our number one priority is your comfort, so we will ensure that you are experiencing as little pain or discomfort as we can provide.

The treatment requires you in a reclined position, as our expert medical aesthetician drags the laser applicator over the treatment area, precisely controlling the depth of the laser treatment, depending on the condition being treated.

How does Contour TRL work?

Like many other laser resurfacing treatments, Contour TRL uses its highly customizable Erbium YAG laser technology to penetrate and resurface the skin, while simultaneously stimulating collagen and elastin.

What makes the Sciton Contour TRL truly unique when compared to other resurfacing lasers is the technology, which allows your treatment provider to have independent control over the depth of ablation and coagulation with micron precision. A micron is equivalent to one millionth of a meter, and is a common unit of measurement used for the skin. With the TRL technology, the precision and control is unmatched by any other laser of this class—with the ability to penetrate as shallow as 4 microns, to as deep as 200 microns per pass.

The next feature of the Contour TRL laser which makes it so unique is that it makes it possible to ablate (resurface) as shallow or deep as is appropriate for the patient, with or without coagulation (heat). Because the Contour TRL decouples ablation from coagulation unlike some other lasers, we can dial in the exact tissue effect needed for each patient’s pathology, objectives, and tolerance for downtime.

Who is a candidate for Contour TRL?

As Contour TRL is our most powerful laser, it is typically reserved for patients with moderate-severe signs of aging, or acne scarring. We have a number of laser treatments that target various conditions, where TRL is the most ideal is if you observe the following skin conditions or concerns:

  • Mild to deep wrinkles
  • Persistent peri-oral (around the mouth) and peri-orbital (around the eyes) deep rhytides
  • Acne scars, post-traumatic scars and surgical scars
  • Actinic keratoses
  • Pigmented lesions
  • Lax skin in all areas
  • Poor skin tone or texture

What should I expect with ProFractional laser resurfacing?

Post-treatment, the wound-healing response already starts to kick in as a natural response to the controlled micro-damage and resurfacing. It is extremely common to experience redness, swelling and pinpoint bleeding and light scabbing directly after treatment.

One day after treatment your skin will begin to flake and peel off naturally, and it is strongly advised to let this happen naturally and don’t exfoliate.

3-4 days from treatment you will continue to flake and peel as your skin continues to recover. Most people tend to take this as time off from work resting, as it is suggested to avoid putting makeup on during this period, and to avoid exercise (very light exercise is fine).

5-7 days from treatment you will be able to return to your normal routine. Mineral makeup can be used to cover redness, and you can resume more strenuous exercise.

The healing process is an important part of ProFractional treatment, as your skin will continue to gradually repair itself over the next few weeks while collagen and elastin are rebuilt in the dermis and fresh, healthy tissue replace the ablated skin. Two weeks post-treatment, once the skin has healed and flaking has completely stopped, you will already notice brighter, fresher and clearer skin—but that isn’t where the miracle stops.

Your skin continues to generate collagen and elastin deep in the dermis for the 6 months following treatment. At the 6 month mark is where your skin will have a significantly improved appearance, and the best of the results will have come to the surface, leaving your skin refreshed, healthy and glowing for all to see!

How is ProFractional laser resurfacing performed?

Once your skin has been numbed and you’ve been prepared in your treatment bed, your ProFractional procedure begins. Your medical aesthetician programs the ProFractional system according to what you discussed in your consultation with consideration of your concerns, desired outcomes, and tolerance for downtime. The device then gets pressed gently to the face in a series of strategic passes, allowing the laser to penetrate micro channels in the skin. With pain properly controlled, your face won’t feel a thing throughout the treatment. Depending on what is being treated, the treatment length can take from 15-30 minutes to an hour.

How does ProFractional laser work?

After your ProFractional treatment, your skin will continue to gradually repair itself over the next few weeks while collagen and elastin are rebuilt in the dermis and fresh, healthy tissue replace the ablated skin. For the best results, ProFractional treatments are typically performed in a series of 3 or more sessions, which are spaced out approximately 6 weeks in between. Upon completion of your treatments and your healing over about 6 more months, your skin will have a significantly improved appearance whether smoothing out acne scars and surgical scars, smoothing deeper wrinkles or age lines, correcting melasma, or fading brown spots.

With the ability to treat deeper into the dermis than many other laser resurfacing technologies, more optimal results can be achieved for more moderate concerns like scar removal and wrinkle smoothing in a smaller number of treatment sessions—without the compromise of an extensive downtime period.

What should I expect with sclerotherapy injections?

After treatment you will be required to wear a compression garment during your recovery for one week. You will be able to return to work the same day, but should take it easy for a few days post-procedure (no strenuous exercise). Complete pre- and post-treatment instructions will be provided to you by your injector.

How do sclerotherapy injections work?

Spider veins are caused when the veins that carry blood back to the heart become damaged or weak. These veins contain a one-way valve that closes once blood has passed through it, ensuring that blood doesn’t flow backwards. If this one-way valve weakens or becomes damaged, which can happen simply by aging, it can result in blood pooling inside the vein. Over time this results in a bulge in the skin that branches out like a spider’s web; thus the name spider veins.

Sclerotherapy treats this by flushing the vein out with a sugar or salt solution, which then causes the blood vessel to collapse and stick together. Over time, the vessel turns into scar tissue and will fade, resulting in the vanishing of the superficial spider vein.

How are double chin injections performed?

Your treatment sessions will require you to be in a relaxed, reclined position. Your treatment provider will mark a series of injection points under your chin, which will be in a grid-like pattern. How is double eyelid surgery performed?

At 8 West Clinic, Dr. Thomas Buonassisi performs two different types of double eyelid surgery; the incisional and suture techniques. Which technique is used depends on a number of deciding factors, and depends on the candidacy of each individual patient. During your in-person consultation with Dr. Buonassisi, the technique that is optimal for you will be discussed.

Incisional Technique: Dr. Buonassisi carefully makes fine incisions in the upper eyelid skin, and removes any excess skin and fat that is necessary. The incision is then sealed using stitches, and the surgical scar is hidden in the new eyelid crease. Double eyelid surgery takes approximately 1 hour, and patients can return home the same day.

Suture Technique: Rather than making an incision and removing fat and skin, the suture technique (as the name indicates) utilizes a number of small incisions, and surgical thread being placed throughout the eyelid to create a crease. The suture remains in the eyelid, and will compress the eyelid skin to create the crease. This technique can be revised and altered at a later date, by removing the suture, and repeating the procedure.

Who is a candidate for double eyelid surgery?

If you were born with eyelids that do not have a crease, or your creases are asymmetrical, and you are bothered with the appearance of closed, puffy upper eyelids, double eyelid surgery may be the ideal option for you.

You may be a candidate for double eyelid surgery if you have any of the following physical features:

  • Single eyelid (no upper eyelid fold)
  • Asymmetrical upper eyelid folds

How is double chin surgery performed?

An incision is placed just underneath the chin where it is easily hidden. The skin of the neck is elevated to expose the underlying fat. If there is extra fat, it is removed either directly or with liposuction. If the patient has a lax platysma muscle, it is tightly bound with sutures. This allows for significant structural support for the neck and a defined neck angle. If the patient’s chin is recessive, a chin implant is used to create a more prominent chin area. The incisions are closed with dissolvable stitches. A compression garment must then be worn diligently for several weeks after the surgery.

Who is a candidate for double chin surgery?

Patients with any combination of features including: a lax platysma muscle, excess fat in the neck area, and/or a recessive chin may be candidates for a double chin surgery. Double chin surgery may be appropriate for younger patients under the age of 40 who don’t have any lower face or neck skin laxity. Patients over the age of 40 typically have lower face laxity that would render double chin surgery ineffective. Patients over the age of 40 who have a poor neck angle or double chin are also typically looking for an improvement in the jowls, jawline, and cheek area—making them more ideal candidates for lower facelift surgery.

How is neck liposuction performed?

For patients who would benefit from liposuction alone, tiny incisions are placed in the neck area where they are easily hidden. Excess fat is gently removed with liposuction. The incisions are closed with dissolvable stitches. A compression garment is worn after surgery to help the skin retract back onto the new neck shape.

Who is a candidate for neck liposuction?

Neck liposuction surgery is ideal for people of all ages who are frustrated with the appearance of a fuller neck that lacks definition. This surgery is not appropriate for patients who are looking for an improvement in the lower face area, such as the jawline or jowls—these patients may be candidates for facelift surgery.

What are the types of rhinoplasty techniques?

For people who want a rhinoplasty, it is tempting to want to focus strictly on how the nose will look after surgery. TIP REFINEMENT & ROTATION RHINOPLASTY:

This term refers to a collection of surgical techniques that target the tip of the nose.   • Dynamic Wrinkles are those caused by making certain expressions; the lines by our mouth when laughing and smiling, the creases on your forehead when you frown or are focusing. These lines come and go with your expressions, but after a while can leave more permanent marks on your face.
  • Static Wrinkles are etched more permanently in our skin, and will remain during a neutral facial expression.

How long does it take to recover from brow lift surgery?

Most patients take at least 1 week off of work to recover from their brow lift. Stitches are removed approximately 1 week after surgery. You’ll need to take 3 to 4 weeks off from vigorous exercise and activities to allow for proper healing and to minimize swelling. You’ll see a significant change right after surgery, but it takes a couple of weeks for the brows to settle into their natural position. How long does a brow lift last? A brow lift can last for years, even up to a decade, but over time, the face and skin will still continue to age as normal.

How is brow lift surgery performed?

Brow lift surgery takes an average of 1-2 hours. How long does it take to recover from blepharoplasty surgery?

We suggest that patients take about a week off of work, and about 2 weeks off from strenuous activities and exercise. Most patients are comfortable being back in the public eye after a couple of weeks, but full recovery takes several months. There are no visible scars from the incisions. You can expect to enjoy your results for at least 10 to 15 years, but as you age, your skin will begin to naturally droop over time.

How is blepharoplasty performed?

Blepharoplasty (depending on the specific technique) typically takes around 1 hour and is performed under local anesthesia. How long does it take to recover from FaceTite and AccuTite?

The first few days after the procedure, patients can expect to have some bruising, swelling, redness and tenderness in the treatment area. It is advised to sleep on several pillows to keep your head elevated for at least 2-3 days to help minimize swelling. Any soreness is easily managed by simply taking Tylenol. It typically takes approximately 7-10 days to see the swelling and bruising subside. It is best to minimize talking and chewing and we recommend following a soft diet for the first 48 hours following your procedure.

You must limit physical activity and exposure to excessive heat (sauna, hot tub, hot yoga, fire pits, UV light) for 2 weeks, and avoid alcohol for the first few days.

While some results will be visible shortly after your treatment, the final results will develop over a longer period of time, often 6-9 months, as the body produces more collagen and tightens the skin in the treated area(s). You will be given a compression facial wrap to wear home immediately following your procedure, and will be instructed to wear this around the clock for a few days, and then you can reduce it to 12 hours a day for the next 2 weeks.

What should I expect with FaceTite and AccuTite?

Your treatment may take up to an hour. The length of your procedure will depend on the size of the treatment area(s). It is performed under local anaesthetic, such as lidocaine, to numb the area. This helps to ensure that you are pain-free and experience minimal or no discomfort during your procedure.

The FaceTite or AccuTite probe is then inserted through tiny incisions (puncture holes) made in the treatment area, generally below the ear and under the chin for FaceTite. Through this probe, RF heat is sent directly to the subdermal tissue. A surface probe is used externally to ensure the most effective delivery of RF heat to the targeted tissue. Once the area has been thoroughly treated with RF heat, the procedure is over. There is no need for stitches, as the necessary incisions are quite tiny. You will be quite swollen and may experience some bruising immediately following the treatment. How long does it take to recover from otoplasty?

Recovery from otoplasty requires you to wear a dressing around the ears for the first two weeks post-surgery. It is normal to experience some bruising, discolouration and swelling around the ears. You can resume most normal activities, such as household chores, a brisk walk, or light exercise within a few days after the procedure. You can resume more vigorous, weight-based exercises once the dressing has been removed.

How is Otoplasty Performed?

Otoplasty takes around 2 hours to perform, and is typically done under local anesthetic. The technique used will vary depending on what is causing the ear to protrude.

Dr. Buonassisi meticulously creates an incision behind the ear, and excess cartilage and skin may be removed. The cartilage is then folded into the proper position, and secured with stitches. Because of their placements, the incisions are virtually imperceptible, for a natural result.

Who is a candidate for otoplasty?

Most people who feel that they have protruding ears are optimal candidates for otoplasty. How chin implant surgery is performed

There are several implant types and insertion methods that can be used to achieve chin augmentation. Dr. Buonassisi carefully evaluates the facial profile and through discussion will recommend the size and shape of implant that would best suit the individual. Most commonly a small incision is strategically placed under the chin, so that any scarring is well-hidden. The soft tissues of the chin are then elevated away from the bone, and a small pocket is created under the periosteum (thick layer that envelops the bone) where the implant is inserted.
