How Do I Receive and Complete My Facelift Consent Forms?

Once you have scheduled your surgery, your patient care advisor will manage all the details of booking your surgery with the surgery center and making sure you are well prepared for your procedure. She will provide several documents that need to be signed or actioned by you. These include 1) facelift consent forms 2) facelift pre and post operative instructions 3) lab requisitions 4) history and physical 5) facelift prescriptions.

Consent forms are essential - they ensure that you understand the risks of facelift surgery that were discussed with you during your pre operative consultation with Dr. Buonassisi. Consent forms are required by the College of Physicians and Surgeons of BC when your surgery is booked with the surgery center.
